| 2011-2012 Tryout forms The following forms must be received at the time of registration: o Mt. Lebanon Hockey Association 2011-2012 Registration Form o USA Hockey – Waiver of Liability o USA Hockey – Consent to Treat/Medical History Form o USA Hockey – Code of Conduct Form o Two separate checks payable to MLHSHA as indicated below: § Non-refundable tryout fee of $100 for Varsity and JV Tryouts and $75 for Freshman Tryouts § Refundable deposit of $300 for Varsity, JV and Freshman to be credited to the total annual hockey tuition. o A copy of your player’s birth certificate, if not already a member of a MLHSHA Team. If you played for the organization previously we will have one on file. o YOU MUST REGISTER AND OBTAIN YOUR PLAYER’S INDIVIDUAL MEMBERSHIP REGISTRATION ONLINE AT www.usahockeyregistration.comPRIOR TO TRYOUTS, AND INCLUDE A COPY OF IT WITH YOUR PLAYER’S REGISTRATION. o Please note that the USA Hockey Registration runs from May 1st 2011 until May 1st 2012. When registering, make certain that you are registering for the 2011 – 2012 year. Please bring a copy of your IMR to tryouts. The above items are required before any player can participate in any on-ice activities, including tryouts. *Players at all three levels (V, JV, Fr) must turn in a physical form by August 5, 2011. We are using the Section #4 physical form listed on the "mtlsd" website. The player should fill out Section #3 for the physician to use, but Section #3 is not a required form for the association, only section#4. The physical form can be delivered to each team's current business manager.
** If a player's 1st tryout is the supplemental tryout, the physical form needs to be turned in at that tryout.
***All physicals for the 2011-2012 school year, MUST be dated after June 1, 2011***
