Please Note - Bring a copy of the team roster and all coaching cards to this meeting.  We will be reviewing cards to ensure that coaches have met the coaching CEP requirements prior to the December 31st deadline.

Dearborn Ice Arena

1. Check in the amount of $250 payable to MAHA D2 (N/A for AAA or Girls)
2. Team Roster (Certified copy)
3. Copy of government issued birth certificate, or, if birth certificate is not available, copy of a passport ID Page for each rostered player.  Non-US citizen must provide proof of legal residency and proof of release and transfer from home country Ice Hockey Federation.
4. USA Hockey Consent to Treat form for each player, coach and manager.
5. Game log showing all games played (League, Scrimmage & Tournament).
6. Score sheets for all games played.
7. Coaching Education Program cards for all Coaches.
8. Signed Waiver of Liability Release for each player, coach or manager who is not electronically registered.
9. Completed Participant Code of Conduct for player, coach or manager.
10. Star program waivers signed by each parent.
11. Background Screening confirmation form for Coaches and Manager.

Note:  "Student Coaches must sign a hard copy of the Coaching Ethics form which must be turned in to the District Registrar.  All other coaches, the Ethics form is now part of your Background Check.
