SAHOF STATE CHAMPIONSHIPS 2011-12

If your team has already qualified or is close to qualifying, In order to secure your spot you will need to provide the following to the SAHOF Tournament committee. Any team that does not qualify will receive a full refund of their entry fees.

1. Check made out to SAHOF Hockey
2. Certified USAH Roster you can fax this to 954-919-9935 or scan as a PDF and send to the email below.
3. Team Roster in either an excel or word file. Include coaches and manager. PLAYERS name, number and position. Use one of these forms:Team Roster-Excel-03 or Team Roster-Word
4. TEAM Photo, this must be in jpeg format approx 4x 6, this is for the program so the better the picture the better it will look.

5. ALL DOCUMENTS, PAYMENT AND TEAM PHOTOS ARE DUE:
