Selected players will receive email invitations by Thursday May 12, 2011 from info@usahockey.com. This e-mail will contain a username and password, that will need to be used to register for the 2011 National Player Development Camp. The e-mail will also contain a link to the Participants Guidebook. The Participants Guide book contains very important information on camp details, personal information, booking travel, location of the camps and most questions can be answered by reading this document. The Player/ Guardian /Parent will need follow the steps in the e-mail to register. All information including travel plans and required forms must be completed at this time. Players need to complete the registration no later than June 1, 2011. If players do not register by this date, it may result in losing their position at camp. These e-mails sometimes find their way into your Spam folders, so please check these folders. If you have not received an email invitation by Friday May 13, 2011, please contact Jodie Penn at the USA Hockey National Office,Jodiep@usahockey.org. Please provide Jodie with the players name, DOB and a correct e-mail address to resend the invitation. In order to complete registration, all forms need to be signed and sent back to the USA Hockey National office, information on this is provided within the online registration tool. |
