If you are a PNAHA referee, coach, manager or volunteer and you reside in Canada you are required to perform the Acxiom online registration, but in addition you will be required to manually complete a Canadian Release and International Service Agreement as well. To download the Canadian Release and International Service Agreement click on the links below. Complete the Canadian Release and International Service Agreement and submit them to your local association Screening Point-of-Contact along with a certified check or money order in the amount of $49.00. Your local association Screening Point-of-Contact will fax the form to the PNAHA President. Currently PNAHA in cooperation with USA Hockey is completing an agreement with Hockey Canada that would require Canadian residents to submit to a Canadian Coaching Screening as well to assure that Canadian Citizens are subjected to the same level of screening as U.S. Citizens are. Canadian Citizens are responsible for all additional costs involved in completing their screening process. Failure to agree to submit to the Canadian Coaching Screening will immediately result in your dismisal as a PNAHA volunteer. Any coach, manager, referee or other individual who is required to be screened per PNAHA's SCREENING POLICY who continues to officiate, coach, manage or otherwise participate in any PNAHA activity without the proper screening certification will be referred to the PNAHA Disciplinary Committee for appropriate action.
