The following forms are required by PNAHA to complete the Screening process. Each of these forms are to be completed and returned to your local association. The PNAHA Application/Screening Disclosure Statement (Part A) is required by both Federal/State Law. Additionally, (Part B) is required by the State of Washington and the Washington State Patrol as a condition of the WATCH Program.

Please complete these forms and submit them to your local association screening point-of-contact. You will also be required to check Liability and Adverse action boxes when you register online.

Canadian Release (This form is only required if you are a Canadian resident and officiating, coaching, or managing PNAHA teams)
