| Player / Parent Registration Forms The following items will be needed to completely register a player with the MHA for a season. No signatures are required by players born in 2002 or later (Mini-Mite and Mite). We will issue the required forms (bullets 2, 3, 4 above) to each team's Manager in the fall. Teams will be responsible for collecting these forms from all players. These forms will be needed later in the season when "Credentials" meetings are held in advance of MAHA playoffs. All of the required forms can be accessed above or directly through the MAHA website.
