Donation Requests   

The USA Hockey Foundation provides charitable organizations with items to be used during fundraising activities, such as auctions or raffles. The Foundation fulfills requests from organizations that meet all of the following criteria:
 

1) USA Hockey registered organizations  

2) 501(c)(3) organizations  

3) Organizations that support the growth of hockey 
 

To receive consideration of your request, please send an email with your name, mailing address, phone number and event information to Sheila May. All requests must be received at least three weeks prior to the event date.  You may also send the above information via mail to:
