2007-2008 Season

• Home teams are responsible to report all games by Tuesday after the weekend. Please fill out the form and ensure that all HFB, Misconducts, and Game Misconducts are listed.
• Game Misconduct requires a player disqualification of the event and the next scheduled game of that team.
• Please Fax the "Bantam Game Reporting " form to 207-512-1092.
• Please Fax the "Peewee Game Reporting " form to 207-512-1092.
• Please Fax the "Squirt Game Reporting " form to 207-692-1016.

• MeAHA will post the forms that all the Competitive Teams will use throughout the year with 2 exceptions, the "Consent to Treat" and the "Waiver of Liability" forms will not be posted. 
• The reasoning behind this is that these forms are legal documents and get changed sometimes on a yearly basis.  Then the wrong form is used during the following year and then it turns into a legal issue.  You will notice that you cannot find these forms on the USA Hockey website either.
• You can acquire the "Consent to Treat" and "Waiver of Liability" forms from your local registrar.

Code of Conduct 
Tournament Application - The tournament app needs to be filled out and submitted to the state registrar, be sure to include the check made out to USA Hockey.
Travel Permit - The canadian travel permit needs to be submitted to the state registrar (or fax to 873-5966) for processing.  Here are the instructions for filling out the form.  Allow a minimum of one week for processing.
