The Indiana Youth Hockey Association along with First Commonwealth and the McCoy family have created a unique opportunity for children to participate in Indiana Youth Hockey Association (IYHA) travel hockey by providing scholarships to offset player fees and to assist families with financial needs.
IYHA has contracted with FACTS to manage the scholarship fund in order to provide complete confidentiality and to ensure that the funds are distributed strictly based on a financial need basis. Because FACTS is geared towards educational financial aid, the form is not modified for our hockey association. Therefore, the financial amount requested should not exceed the player fees.
FACTS Grant & Aid Assessment will be conducting the financial need analysis for IYHA for the upcoming 2008-2009 hockey season. Families applying for financial aid will need to complete an application and submit the necessary supporting documentation to FACTS Grant & Aid Assessment by August 15, 2008. Paper applications will be available by contacting Tom Petro or Phyllis Bartholow. Applicants can apply online beginning May 1, 2008 at www.factstuitionaid.com. FACTS Grant & Aid Assessment will provide a self-addressed envelope for you to mail the following information:
- Signed and completed paper application.
- Payment of the $20 application fee.
- Copies of your most recent tax forms including all supporting tax schedules.
- Copies of your 2007 W-2 form for both you and your spouse.
- Copies of supporting documentation for Social Security Income, Welfare, Child Support, Food Stamps, Workers Compensation, and TANF.
