For Players:

1. Med02 Revised (1 per player) – this is the updated physical form.  Please only submit the 2nd page.  GNASH can not accept any medical information on a player instead we just need to have a signed statement from the physician stating the athlete is cleared to play.  [Form attached]
2. Code of Conduct (1 per player) – signed by player and parents [Form attached]
3. Consent to Treat  (3 per player) – a copy is also retained at each rink [Form attached]
4. USA Hockey Registration confirmation page (1 per player) – make sure it is for 2007-2008 - Valid Sept 1, 2007 - Aug 31, 2008 [to be completed online at www.usahockey.com]

For Team:

1. GNASH Academic Eligibility (1 per team) – must list all players and be signed by principal, assistant principal or athletic director. [Form attached]
2. Team Roster (1 per team) – this must list all players and coaches with numbers and positions. [Form attached]

For Coaches:

1. Code of Conduct (1 per coach)
2. Background Check (1 per coach) – submitted directly to SAHA as noted on the form. [Form attached]
3. USA Hockey Registration confirmation (1 per coach) – – make sure it is for 2007-2008 - Valid Sept 1, 2007 - Aug 31, 2008 [to be completed online at www.usahockey.com]
4. CEP Level 3 or above confirmation (1 per coach) – a copy of the current valid card is sufficient [registration to be completed online at www.usahockey.com, you must attend a class in order to receive the required certification and subsequent card]
