1.  Register online with USA Hockey at www.usahockey.com by following the instructions provided. You should receive and print a bar-coded  registration confirmation when complete.

2.  Download and complete these forms:

2009-2010 CAHA Registration Form

CAHA Player Code of Conduct Form  

CAHA Parent Code of Conduct Form 

USA Hockey Consent to Treat Form  

USA Hockey Waiver of Liability Release Form  

USA Hockey Code of Conduct Form

3.  Mail these completed forms, your bar-coded USA Hockey registration confirmation, copy of player birth cert., and a $100 check (payable to CAHA) to: CAHA, PO Box 1023 Cadillac, MI 49601.

4.  Or you may register in person at the Wexford County Civic Center on September 9 or 10, 2009 from 6-8pm.
