For a Current List of Coaches, Managers and Referees
who have complied with the AHAI Screening Policy

Effective immediately the screening policy for AHAI members is a FULL SCREENING option only. In the interests of our players the AHAI Board has unanimously decided to discontinue the random screening option. When the original screening program began, it was understood that the cost to screen all coaches and managers in a single season would be prohibitive for most clubs. Now clubs have more a maintenance expense for screening. To that end we will offer only the FULL SCREEN option to better secure our participants.

All Tier I, Tier II, Girl's, High School and House League Coaches and Managers must be screened by following the process outlined below or they will be ineligible to participate in any team activities until they have completed the screening requirements.

Requests for Screening Forms will only be processed if they are sent by the Club, School, or Organization President. Please print out the Coach's/Manager's List Form, complete it and send in with applicable fee to address listed below.

Requests for Screening Forms from individuals will not be processed and will be returned.

To obtain a Screening Receipt Form, you must send a check made payable to AHAI in the amount of $58.00 for each person you wish to have screened. Send your check made payable to AHAI and mail to:

Please include the full name of the individual(s) you wish to be screened and a return address as to where to mail the screening forms. These forms will be mailed within 24 hours of receipt of your request and full payment.

Any Tier I, Tier II, Girl's, High School or House League Coach or Manager who continues to coach or manage without the proper screening certification will be referred to the AHAI Rules & Ethics Committee for appropriate action.

NO REFUNDS ARE ISSUED WITHOUT RETURN OF THE COMPLETE SCREENING PACKET ISSUED AND NO REPLACEMENTS ARE ISSUED FOR ANY LOST, MIS-PLACED OR STOLEN SCREENING PACKETS.
